Please be aware that we have a 24-hour Cancellation and No-Show policy. If you cancel less than 24-hours or are a no-show for your appointment, your credit card will be charged $50.
We are no longer a provider for Cigna, ASH, Anthem, Blue Cross, and Blue Shield. If you are unsure if your treatment is covered by your insurance policy, please call your insurance company and our office.
New Patient - Acupuncture and/or Frequency Medicine
Age 13 - 99+
For new patients who are scheduling an appointment for the very first time with our clinic.
Includes consultation and first acupuncture medicine treatment. This is also required for all new patients interested in injection therapies.

New Pediatric Patient - Acupuncture Medicine
Children 1-12
For pediatric children under the age of 7, our practitioner uses a pediatric acupressure technique called Shoni-shin. This is a method of massage, tapping and pressure points instead of using needles.
As a child ages, they become more responsive to acupuncture medicine than not Shoni-shin techniques. Our practitioner can accurately test to see which method is best for your child.

New Patient - Cupping
For new patients who wish to schedule an appointment for cupping only. This also includes a brief consultation.

Acupuncture/Frequency Medicine - Follow-up
Ages 13 - 99+
For patients who have already been treated by our practitioner in our clinic, or for those patients who have already received an initial consultation.

Acupuncture Medicine for Pediatrics - Follow-up
Ages 1-12
For children who have already been seen by our practitioner in our clinic.

Vision Treatments - Acupuncture Medicine for Eye & Vision Health
Dr. Blazek specializes in acupuncture medicine for eye & vision health. Her specialty training by Dr. Andrew Rosenfarb and AcuVision helps her successfully treat vision health for conditions such as Glaucoma, Macular Degeneration, Retinitis Pigmentosa, Diabetic Retinopathy, Cataracts, Dry Eye, etc.
Patients are required to have a patient consultation before a series of treatments can be scheduled.

Injection Therapies
ProloTherapy, FDA approved Biotherapeutics, Saline, Caine injectables for trigger points, perineurial injections, natural vitamins and minerals.

Patients are required to pay for their appointment at the time of service until insurance coverage is verified with an EOB. Once insurance verification is done, payment will be applied towards co-pays or co-insurance and in some cases refunded if applicable.

Some insurances may only provide partial payment and/or specify a patient deductible be met prior to payment. A co-pay and/or co-insurance may also be required. We are not responsible for mistakes made by your insurance provider. You are responsible for all payments on your account.

Most insurance providers do not cover injection therapies such as Prolotherapy, Ozone and trigger point injections and herbal or nutritional supplements. In most cases this is an out-of-pocket expense.

Vision Treatments are not covered under insurance. This is a specialty treatment and does not fall under normal acupuncture therapy.
A VA and TriWest Community Care Provider
We are not an in-network provider for Cigna, ASH, Blue Cross and Blue Shield.
We are not a provider for Medicare, Medicaid or Tricare.
However, if you have secondary or supplimental insurance, your treatment might be covered. Please call our office for more information.

Preparing for your appointment

You should receive an email with your appointment date, time and location. If you do not receive an email, please call our office.

If you are a new patient, please make sure you follow the instructions in the email to fill out your forms online. If you show up at your appointment time to fill out forms, this will take time from your acupuncture treatment.

Please fill out your heath forms online to save time. Otherwise, arrive 20-30 minutes early for your first appointment.

Appointments usually last between 40-50 min. If necessary, sometimes they last longer. Please let us know if you need to leave early.

Eat something lite before your appointment.

Wear clothing that is stretchy and can be moved up to 6" above the knees, elbows, shoulders etc.

Women, wear a regular bra, not a sports bra.

No skinny jeans please.

Remove any watches, wrist jewelry or large earrings.

Bring a list of any western medications, vitamins, herbs or other supplements that you are currently taking, the dosage amount, and the number of times you take per day/week.

DO NOT BRING CHILDREN OR PETS to your appointment. You will be lying down with several needles in and it can be dangerous to move around as the needles can shift, become lodged, bend or even fall out. We have no way to watch your children or pets for you. Animals used for special needs such as physical assistance, seeing-eye dogs, etc., are an exception.

Turn off all cell phones and electronics during your treatment. Ringing phones can disturb other patients.

If you arrive more than 15 minutes late for your appointment, your treatment may be cancelled or limited to the time remaining of your appointment. This is done at the provider's discretion.
