Insurance:

If we are in network with your insurance, you are required to pay for service until insurance verification is confirmed with an Explanation of Benefits (EOB). Any balances will be applied towards co-pays or co-insurance, and in some cases refunds may be applicable. Exception to this policy is VA patients.
 
Some insurances may only provide partial payment and/or specify a patient deductible be met prior to payment. A co-pay and/or co-insurance may also be required.
We are not responsible for mistakes made by your insurance provider. And you are responsible for all balances on your account. Please contact your insurance provider with coverage questions.

Most insurance providers do not cover injection therapies such as Perineural Injection Therapy (PIT), Prolotherapy, Ozone, IM vitamins & minerals, homeopathies, and trigger point injections and herbal or nutritional supplements.
In most cases this will be an out-of-pocket expense. Including VA patients, with some exceptions.

We are NOT an in-network provider for Cigna, ASH, Blue Cross, Blue Shield, BCBS or any other ASH services.
We are not a provider for Medicare Part A, Part B, Medicaid or Tricare/Tricare for Life. However, if requested, we will provide a superbill if your insurance accepts them.

If you have secondary or supplemental insurance, including Medicare Part C, your treatment might be covered.
