At this time, Dr. Kathleen Blazek is no longer accepting new or returning VA/CCN referral appointments.
Dr. Lockwood and Dr. Johnston continue to welcome both new and established VA/CCN referral patients.
If you are scheduling online with a VA/CCN referral, please select either Dr. Lockwood or Dr. Johnston and choose the appropriate VA appointment option.
