At this tim Dr. Kathleen Blazek is no longer scheduling VA/CCN referral appointments at this time.
Dr. Lockwood and Dr. Johnston are continue to accept both new and existing VA/CCN referral patients.
If you are scheduling a visit online with a VA referral, please schedule with one of the two practitioners who are accepting VA patients and use the appropriate VA appointment options.
