Payment & Insurance Policy

Payment at Time of Service


All patients are required to pay for their appointment at the time of service.

Exception: VA/CCN patients on a current referral.

​

Missed Visit & Cancellation policy

 

Your appointment time is reserved exclusively for you. A late cancellation or missed visit creates a gap in the schedule that could have been offered to another patient in need of care.

​

To ensure fairness and optimal use of time, we kindly request that you provide at least 24 hours' notice for any cancellations or changes to your appointment.

  • Appointments cancelled with less than 24 hours’ notice will be subject to a $50 late cancellation fee.

  • Missed appointments (“no-shows”) will also incur a $50 fee.

We appreciate your understanding and cooperation in helping us maintain a high level of service for all our patients.

Insurance Information

In-Network Insurance:
  • UnitedHealthCare and Veterans Administration

  • Community Care Network


If we are in-network with your insurance, you are required to pay for your service until we receive verification through an Explanation of Benefits (EOB).

  • Any remaining balances will be applied toward co-pays or co-insurance.

  • Refunds may apply in some cases.

  • Exception: VA/CCN patients.

  • Coverage Limitations:
    Some insurance plans may:

    • Only cover a portion of the treatment.

    • Require that your deductible be met prior to issuing payment.

    • Include co-pays or co-insurance.

  • Patient Responsibility:
    You are responsible for all balances on your account.
    We are not responsible for any errors or miscommunications made by your insurance provider.
    For questions about your specific coverage, please contact your insurance provider directly.

Auto Insurance & Workers’ Compensation


We do not bill auto insurance or workers’ compensation claims directly.

  • Full payment is required at the time of service.

  • We will provide you with detailed receipts to submit to your insurance or employer for potential reimbursement.

Services Typically Not Covered by Insurance


Most insurance plans do not cover the following therapies or supplements. These will generally be out-of-pocket expenses, including for VA patients (with some exceptions):

  • Perineural Injection Therapy (PIT)

  • Prolotherapy

  • Ozone therapy

  • Intramuscular (IM) vitamins & minerals

  • Homeopathic remedies

  • Trigger point injections

  • Herbal or nutritional supplements

Insurance Plans We Do Not Accept

​

We are not in-network with:

  • Cigna

  • ASH

  • Blue Cross / Blue Shield / BCBS

  • Any other ASH-affiliated plans

We are not providers for:

  • Medicare Part A or B

  • Medicaid

  • Tricare or Tricare for Life

However, upon request, we are happy to provide a superbill if your insurance accepts them.

Secondary or Supplemental Insurance


If you have secondary or supplemental coverage, such as Medicare Part C, your treatment may be eligible for reimbursement.
